AI voice agents for hospitals in India: what they can and cannot do

What an AI voice agent can take off a hospital's appointment line today, where it should hand over to people, and the questions to ask before choosing one.

Hospitals in India are trying AI voice agents for the same reason clinics are: the appointment line is busiest exactly when staff are busiest. Here is a plain account of what these agents do well, what they should not do, and how to judge one.

What a voice agent can do well

  • Answer every call, in the caller's language. Hindi, Tamil, Telugu, Kannada, Marathi, Bengali and others, including callers who switch mid-sentence.
  • Send the caller to the right department. From the complaint the patient describes, not a menu they have to navigate.
  • Respect each doctor's hours. It should never offer a slot a doctor does not sit, or quote a fee from memory.
  • Remind patients. WhatsApp reminders before the appointment cut no-shows and let patients reschedule by replying.

What it should never do

  • Give medical advice, a diagnosis or a dosage, however the question is put.
  • Handle an emergency itself. A caller describing chest pain or breathlessness should be told to go to emergency or call 108, first.
  • Pretend to be human. It should say it is an AI at the start of the call.
  • Read out reports or results.

Voice agent, IVR or call centre?

An IVR routes calls by key presses and still needs a person at the end. A call centre works but costs seats on every shift. A voice agent finishes the routine booking itself and passes the rest to your team with the transcript. Many hospitals keep their call centre and use the agent for overflow, lunch hours, nights and WhatsApp. There is more on this in AI voice agent vs IVR.

Questions to ask before choosing one

  1. How does it decide the department from a complaint? Ask it about "acidity" and see whether it sends you to cardiology.
  2. What happens when a caller asks for a person?
  3. Which languages are live today, not on a roadmap?
  4. How are prices set: per call, per minute or per doctor?
  5. Can your team read every transcript the same day?
